This policy covers this website only. How the practice handles your health information as a patient is described in the practice's Notice of Privacy Practices, which you can request from the office.

Information you send us

If you use the contact form, we receive the name, email address, phone number, state, reason and message you enter, along with the time, the page you sent it from, how you reached the site, your IP address, and your browser and device type. This is saved on the server that hosts this website, where the office reads it, and may also be sent to the office by email through a third-party email delivery service. We use it only to reply to you and to keep spam out. Please do not send health information through the form or by email; they are not secure medical channels.

Information collected automatically

We use Google Analytics to understand how visitors use the site, for example which pages are read and whether the phone number is tapped. It uses cookies and collects information such as your approximate location, device and browser. Advertising features are turned off. You can opt out with Google's browser add-on or your browser's cookie settings.

So that the office can see how people find the practice, the site keeps a short note in your browser's storage for up to 90 days: the first page you arrived on, the site that sent you here, and any campaign tags in the link. It stays in your browser and is sent to us only if you submit the contact form.

Our hosting provider keeps standard server logs, such as IP address and pages requested, for security and reliability.

Sharing

We do not sell or rent personal information, and we do not share it except with the service providers that run this site and deliver its email, or when required by law.
