Key points

  • Part B covers outpatient psychiatric evaluation and medication management; in 2026 the deductible is $283, then you pay 20 percent of the approved amount.
  • The Part D out-of-pocket threshold is $2,100 in 2026 and $2,400 in 2027, with the maximum deductible rising from $615 to $700.
  • PAAD charges $5 per generic and $7 per brand prescription for New Jerseyans with 2026 income under $54,943 (single) or $62,390 (married).
  • Senior Gold covers the next income band, charging $15 plus 50 percent of the remaining cost, with a $15 copay after $2,000 (single) or $3,000 (married) out of pocket.
  • Medicare's in-person condition for mental health telehealth applies only to services after December 31, 2027, but New Jersey's P.L.2026, c.40 in-person rules for stimulants already apply.

Medicare covers adult ADHD care the way it covers other outpatient mental health care: visits under Part B, medication under Part D. What makes New Jersey different is a layer of state help on top. PAAD and Senior Gold lower prescription costs for many older adults and for adults on Social Security disability, and New Jersey's July 2026 telehealth law sets in-person visit rules for stimulant patients no matter who pays. This article covers all three, plus the questions to settle before a first visit. Medicare's Open Enrollment Period, October 15 to December 7, is the time to fix a drug plan that does not fit.

The clinical side of a later-life diagnosis is covered in ADHD in adults over 50. Whether Dr. Shershow's practice works with Medicare is a question for the office; call to ask or see the fees and insurance page.

Part B: the evaluation and follow-up visits

Medicare Part B covers outpatient mental health care. Medicare lists psychiatric evaluation, medication management and diagnostic tests among covered services, from psychiatrists and other doctors, clinical psychologists, nurse practitioners, physician assistants, clinical social workers and certain other professionals. After the Part B deductible, you pay 20 percent of the Medicare-approved amount when the clinician accepts assignment.

2026 Part B figureAmount
Deductible$283
Standard monthly premium$202.90
Your share after the deductible20% of the Medicare-approved amount

Check medicare.gov for the 2027 Part B amounts before the new year. A Medigap policy may pay some or all of the 20 percent. A Medicare Advantage plan replaces this structure with its own copays and its own network.

Which clinicians you can use

  • Accepts assignment: you pay only the deductible and coinsurance.
  • Non-participating: may charge more than the approved amount; Medicare says in many cases the charge cannot be more than 15 percent above it.
  • Opted out: you pay the full fee under a private contract, and Medicare will not pay, except in emergencies.

Before booking with any private psychiatrist on original Medicare, ask: "Have you opted out of Medicare?" On Medicare Advantage, ask the plan whether the clinician is in its network and whether visits need a referral or prior authorization.

If you are under 65

If you have Medicare before 65 because of a disability, the same Part B and Part D rules apply. New Jersey's PAAD and Senior Gold programs, described below, are open to residents ages 18 to 64 who receive Social Security disability benefits and meet the income limits, so check them even if you think of them as programs for seniors.

Part D: ADHD medication

Drugs you take at home are covered by Part D, either through a stand-alone drug plan or a Medicare Advantage plan with drug coverage. Each plan has its own formulary, tiers and rules, so whether your ADHD medication is covered, and whether it needs prior authorization or has a quantity limit, depends on the plan.

Part D standard benefit20262027
Maximum deductible$615$700
Out-of-pocket threshold (after this, covered drugs cost $0 for the rest of the year)$2,100$2,400

The 2027 figures come from CMS's April 6, 2026 rate announcement. During Open Enrollment, enter your exact medication and pharmacy into the plan finder on medicare.gov and compare the full-year cost, not just the premium. Changes take effect January 1.

New Jersey's help with drug costs: PAAD and Senior Gold

New Jersey runs two prescription programs through the Division of Aging Services. Both are for New Jersey residents who are 65 or older, or 18 to 64 and receiving Social Security disability benefits, and both require Medicare-eligible members to enroll in a Part D plan.

PAAD works alongside your Part D plan, not instead of it. If your Part D plan does not cover a drug because it is not on the formulary, PAAD says you will need to switch to a covered drug or have your prescriber ask the Part D plan for an exception. The state also says both programs are required by law to substitute generics for brand-name drugs. The hotline for both programs is 1-800-792-9745.

What to do during this Open Enrollment

Medicare's Open Enrollment Period runs from October 15 to December 7, 2026, and changes take effect January 1, 2027. If you take ADHD medication, use the window this way:

  • Look up your exact medication and pharmacy in the medicare.gov plan finder for 2027 and compare the full-year cost.
  • Check whether the plan lists prior authorization or a quantity limit for your medication.
  • If your income is near the PAAD or Senior Gold limits, call 1-800-792-9745 and ask how the program works with the plans you are comparing.
  • If the choices are confusing, book a free session with SHIP at 1-800-792-8820.

Telehealth: two separate sets of rules

Medicare's payment rules. Mental health telehealth at home is a permanent Medicare benefit. The law also attaches an in-person visit condition to it, but that condition has been delayed. According to the CMS telehealth FAQ updated in October 2026, it applies to services after December 31, 2027. As of October 2026, no in-person visit is needed before a Medicare telepsychiatry visit.

New Jersey's prescribing rules. Separately, P.L.2026, c.40, effective July 8, 2026, applies to adult patients located in New Jersey who start a Schedule II stimulant by telehealth. They need an in-person visit within 30 days, a contact at least every 3 months (in person or by real-time audio-video), and an in-person visit at least once a year. Medicare paying for a video visit does not change this. As of October 2026, federal Drug Enforcement Administration telemedicine flexibilities for controlled substances run through December 31, 2026. See telehealth ADHD prescriptions in New Jersey.

New Jersey's state insurance protections, such as the in-plan exception rule and the Independent Health Care Appeals Program, do not apply to Medicare. The Department of Banking and Insurance says Medicare has its own appeal system. For how state law treats other plans, see does insurance cover adult ADHD care in New Jersey.

Questions to settle before your first visit

  1. Do I have original Medicare or a Medicare Advantage plan?
  2. Does the clinician accept assignment, or have they opted out of Medicare?
  3. Have I met this year's Part B deductible, and do I have Medigap or other coverage for the 20 percent?
  4. Is my ADHD medication on my Part D formulary, and does it need prior authorization?
  5. Do I qualify for PAAD or Senior Gold, and have I applied?
  6. If I use video visits from New Jersey and take a stimulant, how will the in-person visits be arranged?

How Dr. Shershow's practice arranges visits is described on the appointments and telehealth page, and the office will explain how visits are arranged for patients located in New Jersey.

Questions

Does Medicare cover an adult ADHD evaluation in New Jersey?

Yes. Part B covers outpatient psychiatric evaluation as mental health care. After the Part B deductible you pay 20 percent of the Medicare-approved amount if the clinician accepts assignment. If the psychiatrist has opted out of Medicare, Medicare pays nothing.

Can I use PAAD if I am under 65?

Yes, if you are 18 to 64 and receive Social Security disability benefits, live in New Jersey and meet the income limit. For 2026 that is less than $54,943 if single or $62,390 if married. Medicare-eligible members must also enroll in a Part D plan.

Does PAAD cover a drug my Part D plan will not cover?

Not automatically. The state says that if a drug is not on your Part D plan's formulary, you will need to switch to a covered drug or have your prescriber request an exception from the Part D plan. Call the hotline at 1-800-792-9745 with specific questions.

Do I need an in-person visit for Medicare to pay for video visits with a psychiatrist?

Not as of October 2026. Medicare's in-person condition for mental health telehealth applies to services after December 31, 2027. New Jersey's separate law still requires in-person visits for adults who start a Schedule II stimulant by telehealth.

Who can help me compare Medicare drug plans for free?

New Jersey's State Health Insurance Assistance Program (SHIP) offers free, unbiased counseling at 1-800-792-8820. Counselors do not sell or recommend specific plans.
