Key points

  • The Focalin label describes dexmethylphenidate as the d-threo enantiomer of racemic methylphenidate hydrochloride.
  • Ritalin tablets are labeled for ADHD in pediatric patients 6 and older and adults; Ritalin LA is labeled for children 6 to 12 only.
  • Focalin XR's label includes a placebo-controlled trial in 221 adults aged 18 to 60; Focalin IR trials were in ages 6 to 17.
  • Strength numbers are not interchangeable between the two; the Focalin labels give specific switching instructions for prescribers.
  • A 2018 network meta-analysis grouped dexmethylphenidate with methylphenidate and did not compare them separately.

Focalin and Ritalin are closely related. Ritalin contains methylphenidate, which is a mixture of two mirror-image forms of the molecule. Focalin contains dexmethylphenidate, which the FDA label describes as "the d-threo enantiomer of racemic methylphenidate hydrochloride." Both are Schedule II stimulants in the methylphenidate family. This page compares their FDA labels, including the immediate-release and extended-release versions, as of October 2026.

Same family, different molecule mix

Methylphenidate exists as two mirror-image forms. Ritalin and most methylphenidate products contain both (a racemic mixture). Focalin contains only the d-threo form. One practical consequence is that the strength numbers printed on Focalin and Ritalin are not interchangeable. Both Focalin labels give specific instructions for people switching from methylphenidate, so any switch should be worked out by the prescriber rather than by matching the numbers on the bottle.

The labels do not claim that the single form works better or causes fewer side effects. Both Focalin labels carry the same warnings as Ritalin: abuse and misuse, serious heart disease, blood pressure and heart rate increases, psychiatric effects, priapism, circulation problems in fingers and toes, acute angle-closure glaucoma, increased eye pressure, and tics. All three are contraindicated with MAOIs or within 14 days of stopping one.

What the labels say about adults

This is where the products differ most.

  • Ritalin tablets name adults directly: "pediatric patients 6 years and older and adults." The label gives adult dosing in divided doses through the day.
  • Focalin tablets are indicated for ADHD without a stated age range, but the trials described in the label were in patients aged 6 to 17, and the label says it "has not been studied in the geriatric population." Adult use is within the indication wording, though adult trial data are not presented in the label.
  • Focalin XR includes a randomized, placebo-controlled trial in 221 adults aged 18 to 60 and lists adult side effects separately.
  • Ritalin LA, the long-acting Ritalin capsule, is labeled only for children aged 6 to 12, so it is not the adult counterpart to Focalin XR.

How Focalin XR is built

The Focalin XR label describes "a bi-modal release profile." Each capsule contains half of its medicine as immediate-release beads and half as enteric-coated, delayed-release beads, so it gives an immediate release followed by a delayed one. The label also describes how people already taking Focalin tablets can move to Focalin XR at the same total daily amount, which, like any change, is a decision for the prescriber.

What the research says

A head-to-head trial of dexmethylphenidate against racemic methylphenidate in adults was not found for this guide. One of the pediatric trials in the Focalin label included a racemic methylphenidate arm, but the label reports Focalin against placebo, not against methylphenidate. The 2018 Lancet Psychiatry network meta-analysis (Cortese and colleagues) grouped "methylphenidate (including dexmethylphenidate)" together, finding that the group beat placebo in adults at about 12 weeks and was less well tolerated than placebo. It did not separate the two. More on the family is in methylphenidate vs amphetamine.

Practical points

  • Timing. Ritalin and Focalin IR are short-acting and taken more than once a day. Focalin XR is a once-daily option within the same chemistry. For a long-acting racemic methylphenidate with adult labeling, see Concerta vs Ritalin.
  • Sleep. The Ritalin label suggests taking the last dose before 6 p.m. for people whose sleep is affected.
  • Swallowing. Focalin XR capsules can be opened and the contents sprinkled on applesauce, according to the label.
  • Generics. The Orange Book lists AB-rated generics for Ritalin tablets, Focalin tablets and Focalin XR capsules.
  • Supply. Dexmethylphenidate and methylphenidate immediate-release tablets did not appear as current shortages in the FDA database on October 19, 2026; methylphenidate extended-release tablets did.

The individual drug pages, Focalin for adults and Ritalin for adults, have more detail.

New York and New Jersey rules that apply to both

All the medications on this page are Schedule II controlled substances, so the same prescription rules apply to either one. In New York, a Schedule II prescription cannot be refilled; when more medication is needed, the prescriber issues a new prescription (10 NYCRR 80.67). New Jersey's controlled dangerous substance rules also bar refills of Schedule II prescriptions (N.J.A.C. 13:45H-7.9). Supply limits, the 30-day window to fill, electronic prescribing and the monitoring-program checks are covered in New York's rules for ADHD stimulant prescriptions and New Jersey's rules for ADHD stimulant prescriptions. Switching from one stimulant to another does not change any of these rules.

Questions worth asking

  • Is there a reason to prefer the single-form molecule in my case?
  • Do I want once-daily coverage (Focalin XR) or short-acting doses?
  • If I switch, how will the amount change, and how will we track the effect?
  • Do I have glaucoma, heart disease or a history of tics?

Talking it through with a prescriber

Dr. Shershow is a psychiatrist who focuses on adult ADHD and is licensed in New York and New Jersey. After an adult ADHD evaluation, he discusses the treatment options that fit, including medication when it is appropriate, and follows up over time. The comparison above is general information from FDA labels and published research. It is not advice to start, stop or switch any medication, and neither drug is the right choice for everyone.

Questions

Is Focalin stronger than Ritalin?

Focalin contains only one form of the methylphenidate molecule, so its strength numbers are not comparable to Ritalin's. That does not make it more effective. The labels give prescribers specific instructions for switching.

Is Focalin approved for adults?

Focalin XR's label includes an adult trial and lists adult side effects. Focalin IR tablets are indicated for ADHD without a stated age range, but the label's trials were in patients 6 to 17.

Does Focalin have fewer side effects than Ritalin?

The labels do not say so. They carry the same warnings, and no adult head-to-head trial was found that compares side effects.

Is there a long-acting Ritalin for adults?

Ritalin LA capsules are labeled for children 6 to 12. Long-acting methylphenidate products with adult labeling include Concerta, and Focalin XR is a long-acting dexmethylphenidate with adult trial data.

Are there generics?

Yes. The FDA Orange Book lists AB-rated generics for Ritalin tablets, Focalin tablets and Focalin XR capsules.
